Adults Service

 

Somerset Partnership NHS Foundation Trust provides specialist mental health services for adults of working age with illnesses such as severe depression, anxiety, psychosis (including schizophrenia and bi-polar disorder) and personality disorder.  A Recovery Care Plan Approach (RCPA) has been adopted to emphasise the involvement of people receiving services and the promotion of independence and wellbeing.

 

The multi-disciplinary community service work closely with service users, carers, the voluntary sector, and other providers of statutory services (ie the Local Authority, Probation Service and Primary Care teams), to develop Care Plans to meet service users and carers individual needs.

 

The service targets work with service users with:

 

  • severe and complex mental health problems;
  • a less severe degree of problem where there are risk behaviours or complex dynamics; 
  • requiring specialist intervention;
  • marked behavioural difficulties as a consequence of mental health problems;

 

 

The teams offer assessment and a variety of interventions that may include specialist community and inpatient services to meet the needs of the individuals referred to the service.
