Somerset's Sustainability and Transformation Plan

Somerset’s NHS organisations and County Council have today (10 November) published the county’s Sustainability and Transformation Plan (STP).

This plan describes why we need to change the way health and social care services are structured and delivered in order to meet the future needs of the people of Somerset.

These strategic, five-year plans are being produced across the country and aim to create services that work better for residents, are financially sustainable and have much greater emphasis on prevention.

The plan sets out a way forward for discussion, rather than fully-developed proposals. Over the coming months there will be discussions with the public, patients, carers and professionals to shape and develop the plans and build a shared vision for high quality, sustainable services.
