What is the Diabetes Intermediate Care Service?
Sitting neatly between the GP surgeries and Acute Hospitals the Diabetes Intermediate Care Service provides support and education for people with Type 1 and Type 2 diabetes.
Who are we?
The team consists of a Service Manager, Nurse Consultant, Diabetes Specialist Nurses, Diabetes Specialist Dietitans and admin support.
What do we offer?
We offer specialist advice, support and education sessions for those with both Type 1 and Type 2 diabetes. These are run by Specialist Nurses and Dietitians who have extensive training and specialise in helping people manage their diabetes. An integral part of our role is raising the awareness of diabetes in the community.
Why have you been referred?
We know that managing your diabetes can be challenging and it is likely that most people with diabetes will need specialist advice at some stage. If this is the case your doctor may suggest you have an appointment with the Diabetes Specialist Nurse or Dietitian.
To help you these specialists can:
- carry out a detailed assessment and offer treatment/management options based on your individual needs
- offer you an individual care plan and promote self care where possible
- provide education/training to individuals, groups, carers and nursing staff in the form of Conversation Maps, DESMOND & DAFNE
- provide you with appropriate literature to increase your confidence at self-managing your diabetes
- provide you with access to one of our Specialist Diabetes Dietitians
How can you access these services?
All services are accessed via your GP Surgery through the e-Referral service, however, if you are housebound and have difficulties coming to clinic then your GP/Practice Nurse can refer direct to us and we will organise a home visit for you.
What are the waiting times?
We aim to provide you with a clinic appointment within 6 weeks. We are now offering the opportunity for telephone consultations, if you find this more suitable. We aim to provide you with an education session appointment within 13 weeks.
