Stroke Services

Somerset Partnership provides community based stroke services for the population of Somerset.  We currently have specialist stroke rehabilitation beds at South Petherton, Shepton Mallet and Williton Community Hospitals.  This is supported in the community by the new Early Supported Discharge (ESD) service for stroke which co-ordinates stroke care services so patients can discharged early and rehabilitated in their own home, reducing hospital lengths of stay. 

We have specialist staff in the fields of nursing, physiotherapy, occupational therapy, speech and language therapy, psychology and dietetics.  We provide longer term support for more complex patients, and review all stroke patients at approximately 6 months after their stroke. 

There are Life After Stroke groups running at each of the community based stroke rehabilitation units.  These are 10 week programmes of structures exercise, peer support and education for both the patient and their spouse or carer.  The service is supported by a Consultant Nurse for Stroke and dedicated stroke services manager.
