Who is it for?
Somerset Partnership NHS Foundation Trust Learning Disability Service is a highly specialist health service for adults with learning disabilities. The service aims to meet the additional and specialist health needs of people with learning disabilities and is complementary to the work carried out by mainstream health services.
We have 4 Community Teams for Adults with Learning Disabilities (CTALD) across the county, with the following professions represented:
We are based with our colleagues in Health, but work closely with the Local Authority social work teams in each of the 4 CTALD areas.
There are 2 separate teams which work alongside the CTALDs:
In addition there is a specialist countywide Rapid Intervention Team (RIT). RIT leads on assessment, treatment and expert intervention for people with LD who have highly complex challenging behaviour and/or mental health needs
The Better Health Team supports annual health checks, works with GPs to help them better support people with LD, provides information, training and support on healthy lifestyles, health and medical conditions to people with learning disabilities and those that support them.
How do I access this service? (Assuming they are eligible)
We accept referrals from service users, carers and professionals. Referrals should be made to the relevant community team.

Emergency Duty Team (EDT)
For urgent out of hours situations the Local Authority Emergency Duty Team can be contacted. This Local Authority out of hours service (available from 17:30 - 08:30) deals with emergency or urgently distressing situations. The team covers the Local Authority's statutory responsibilities under mental health, learning disabilities, child care and welfare legislation. It is provided on an out of hours basis, including weekends and bank holidays.
