Hospital passport

Disability Services containing essential information about a service user. The aim of the Hospital Passport is to provide hospital staff with important information about the service user and their health needs when they are admitted to hospital. This is particularly helpful if an individual needs support with their communication or might be anxious about going into hospital.  Service users can take their completed Hospital Passport with them to hospital for any planned or emergency admissions or outpatients appointments.

Safeguarding Adults at Risk

Adult Safeguarding means protecting a person's right to live in safety free from abuse or neglect. 

The Care Act 2014 requires that each Local Authority (LA) make enquiries or ensure others do so if it believes an adult is/or is at risk of abuse or neglect.  An enquiry should establish whether any action needs to be taken to stop or prevent abuse or neglect, and if so by whom.

All safeguarding concerns identified by Somerset Partnership staff should be referred by telephone to Somerset Direct Adults Team on 0300 123 2224. They will take the full details of the concern and workflow these to the Somerset County Council Safeguarding Adults Team for a Sect 42  (Care Act 2014) statutory enquiry decision to determine whether or not the concerns will be treated as a Safeguarding Adult at Risk enquiry or managed as part of the usual care management of the referring or partner agencies.
