Home Treatment Teams
Home Treatment services are provided to people living in the community who require intensive, daily support and who may otherwise be likely to be admitted to in-patient care.
Who we are
We are based in different localities around the county. Our teams consist of a Team Manager, Community Psychiatric Nurses, Specialist Social Workers, Occupational Therapists, Support Workers, and a Secretary.
The team has quick access to specialist mental health Doctors and psychologists.
What we do
We make an initial assessment to establish each persons needs. This usually takes the form of a face to face meeting with two staff, though this may start with a telephone conversation. Important family members or carers are encouraged to be part of this assessment.
Staff aim to provide intensive home support as an alternative to admission, and also assist earlier discharge by providing intensive home support for a short period after discharge from an in-patient service.
How we work
The service prioritises people aged over 18 who are at risk of admission to an inpatient unit.
After this initial assessment a plan of care is agreed with you and staff visit at agreed times ,sometimes up to three times a day to provide you and your family/carers with support . Medication is often provided for you by the service.
We keep your family doctor updated about your progress.
As you improve we will see you less often and discuss with you what follow up arrangements will be suitable in your case.
How you can you access these services
Referrals to the service are normally received from a person’s GP, but we also accept referrals from other professional, voluntary service workers and self referrals.
The service provides a 24 hour service.
People accepted by the service are usually seen on the same day as they are referred.
