Specialist Day Services

In each locality, the Trust provides:

  • specialist day hospital treatments for older people with severe mental health problems
  • specialist day care for people with dementia, Local Authority funded

Who we are

The Day Hospital Services include Psychiatric Nurses, Medical staff, Occupational Therapist and Psychologist input.

The Specialist Dementia Day Care is provided by specialist Day Care staff.

What we do

The Day Services provide awareness, education and support groups for carers, in addition to the groups for service users.

Day Hospitals provide assessment, treatment programmes and ongoing care planning. Patients with dementia are accepted into different groups from those with illnesses such as severe depression or schizophrenia.

Specialist Day Centres are provided by Somerset Partnership for ongoing social day care in each locality for Older People with dementia who need specialist day care to maintain their independence in the community and to provide relief for carers.

How we work

The Day Services offer an initial six week assessment package for people referred by the GP or other Health and Social Care professional.

People attend the Day Service, usually one day per week, for the assessment period. Health aspects are normally assessed at the Day Hospitals, and following the assessment, a Care Plan is drawn up in agreement with the patient / client and any carer. Care Plan options may include: onward referral to other services, attendance at groups, carers groups, medication, memory strategy groups, continuing attendance at day care.

People with severe degrees of dementia may be supported by ongoing specialist social day care attendance, aimed to maximise their independence skills, provide respite for carers and enable people to cope at home as long as possible.

How you can access these services

The best first step is normally to discuss accessing the service with your GP. Your GP will undertake initial tests and arrange a referral.

Referrals to the service are normally received from a person’s GP who undertakes initial screening tests, but the service also accepts referrals from Local Authority social workers, District Nursing staff, Alzheimer’s Society, and accepts queries and referrals direct from patients and their carers.

Waiting time

All new referrals received from the GP are seen within 3 weeks. GPs can also make emergency referrals.
